White Earth Nation: Partnership with Substance Abuse Programs

White Earth Nation’s SNAP-Education program partners with the Maternal Outreach and Mitigation (MOMs) Program. This outpatient program helps moms gain the skills they need to overcome drug addictions. The SNAP-Ed program is successful because it meets specific nutrition education needs. Clients receive culturally grounded nutrition information to help them follow a healthy diet after achieving sobriety. They also learn to cook for themselves and their children. 

Leveraging Technology to Deliver Tailored SNAP-Ed Messages

UCONN School and Family SNAP-Ed collaborated with the Connecticut Children’s Medical Center in Hartford for this project. This project supports healthy behaviors in high-need children receiving healthcare in the urban Pediatric Emergency Department (PED). When they visit, PED, children and families enjoy a fun online survey about dietary behaviors. Based on their responses, the family receives unique diet and physical activity messages.

West Virginia Kids Coupons Program

The Kids Coupon program gives low income children of all ages $4 in farmers market tokens. These tokens are used to purchase fresh, local fruits and vegetables from a market. The produce is brought to their childcare center, school or community. Children and families participate in nutrition education and food sampling.

High Risk Patients Learn Nutrition Skills

Through collaborations with three key partners, Nutrition Educators have reached a total of 252 high-risk patients in 2015 and 2016 with nutrition education. Patients and their families learn how to shop, cook, and eat healthy on a limited budget.
