This evaluation tool was designed to tracking changes in participant knowledge, attitudes, skills, confidence, and behavior as a result of the Cooking Matters course. It is a pretest/post-test design. The survey also solicits feedback regarding the overall quality of the participant experience, attempts to capture quotes from participants about how the course has affected them, and includes questions that gather participants’ background and demographic information.
